Permanent makeup is a form of tattooing: pigment is placed in the skin to create a makeup-like effect, such as eyeliner, lip liner, or eyebrows. It deserves the same thoughtful decision-making as any tattoo, including a careful consultation and aftercare plan.

How Things Have Changed

Years ago, many artists used traditional tattoo equipment that was not designed for the delicate skin around the eyes. Today, the tools I use are designed for cosmetic tattooing and delicate facial skin. I also take my time with the procedure and use a two-step numbing process to help clients stay comfortable.

Serious complications have been reported with certain uses of RF Microneedling devices, including fat loss. The FDA is evaluating those reports and has said that burns, scarring, fat loss, disfigurement, and nerve damage have been reported in dermatologic and aesthetic procedures. That does not mean every treatment causes these outcomes, but it means the risks deserve a candid conversation.

RF Microneedling is a medical procedure, not an at-home cosmetic treatment. A licensed provider should assess whether it is appropriate for you, explain the benefits and risks, and discuss the device and treatment plan being considered.

Why Device Choice, Depth, and Settings Matter

RF Microneedling devices deliver controlled radiofrequency energy through tiny needles at selected depths. RF Microneedling vs CO2 Laser: Which One Should You Choose?

Have you been trying to decide between RF Microneedling and CO2 laser?

Maybe you have been hearing people raving about RF Microneedling, or conversely been really afraid of it. Maybe you have seen those incredible before and after photos from CO2 laser treatments and you are wondering which one is actually better. After more than 25 years in aesthetics I am going to give you an answer that might surprise you, and the answer is neither, because they do completely different jobs.

We Age In Layers

The reason this gets confusing is that we tend to think of aging as one thing, but the truth is that we age in layers. We lose collagen and elasticity, our skin becomes thinner, we develop pigmentation, the texture changes, and we get crepey skin. The deeper structures of the face also begin to shift and descend.

That is why I absolutely love combining RF Microneedling and CO2 laser in the same treatment. They are not competitors. They are teammates.

What RF Microneedling Does

RF Microneedling works in the deeper tissue. It creates controlled injury and that healing or wound response beneath the surface of the skin, where collagen remodeling and tissue contraction occur. This is why we often use RF Microneedling when we are trying to improve skin laxity, firmness, texture, and overall collagen production.

What CO2 Laser Does

CO2 laser is a completely different animal. CO2 works primarily on the surface layers of the skin. It removes tiny fractions of damaged skin and stimulates new skin regeneration. This is where we see beautiful improvements in that awful crepey skin that we hate, in pigmentation, texture, fine lines, and that brighter, smoother appearance that so many people are looking for.

Why My Answer Is Often Both

So when someone asks me which is better, my answer is often both. RF Microneedling addresses that deeper tissue remodeling, while CO2 laser addresses the surface. When you stack them strategically, you are treating aging from multiple layers at one time, and that is where we often see the magic.

Downtime Is Different

Now let us talk about downtime, because this is where these treatments really are very different. RF Microneedling is one of my favorite low-downtime treatments. When we combine RF Microneedling with companion treatments like PDRN and Alastin’s nectar, most of my clients are shocked at how quickly they recover. In many cases, you are basically back to normal the very next day.

Even when we are treating areas like the chest, the arms, or other body areas that typically heal more slowly, this is still true. If you are not combining those healing therapies, you may notice some small grid-like marks on the skin for a few days, and that is completely normal. But generally speaking, you can return to moisturizer, sunscreen, and makeup the very next day, and for busy men and women that is a huge advantage.

CO2 laser is different depending on how aggressive the treatment is. You are typically looking at about five to seven days of downtime. In my experience, CO2 is not actually painful, not during the treatment or after the treatment. With proper numbing, I do not really feel much of anything during the treatment. What surprises people is not the discomfort. It is the reflection in the mirror afterwards.

You may see significant redness, tiny blood spots, or treatment tracks. The skin can look really shiny and very pink while it is healing, and that is because CO2 is intentionally removing tiny fractions of the skin surface. So you are seeing that healing process happen right there in front of you.

Comfort Is Different Too

With RF Microneedling, most of the work is happening beneath the surface. I have often said that about 80 to 90 percent of the wound healing response action is taking place where you cannot see it, down deep. That is one reason the downtime is so much easier.

Both treatments require about 30 to 45 minutes of numbing beforehand. This is where I may have a slightly unpopular opinion, because I actually think RF Microneedling is more uncomfortable than the CO2 laser. Settings matter, technology matters, and provider technique matters, but generally speaking I find RF Microneedling uncomfortable but very manageable. Personally, I like to take an extra strength Tylenol while numbing, and then I find that the treatment is really very comfortable. CO2 with good numbing, honestly, I barely feel it at all.

Which One Should You Choose?

If you are wondering which treatment you should choose, the answer depends entirely on your goals. If you are looking for minimal downtime, collagen stimulation, skin tightening, and gradual improvement, RF Microneedling is really hard to beat. If you are focused on surface texture, crepey skin, pigmentation, fine lines, and dramatic resurfacing, CO2 laser can be absolutely incredible.

And if you really want my favorite answer, it is not choosing one. It is combining them both as a stacked treatment in the same service. The best anti-aging results rarely come from treating just one layer of aging. They come from treating all of them.

The Bottom Line

For my own skin, I do both. RF Microneedling several times a year for that collagen and skin tightening, and CO2 around my eyes because nothing I have found touches crepey skin and fine lines around the eyes quite like it. Every chance I get, I do them as a combined treatment together.

RF microneedling is one of those treatments that can sound a little intimidating until someone explains what is really happening. I love it because it is not just about getting a quick glow before an event. It is about supporting the structure and quality of your skin over time, which is a very different conversation.

At Wake Up Beautiful here in Orange County, I want clients to understand why a treatment is being recommended, what it is designed to do, and how it fits into a bigger skin strategy. RF microneedling is one of my favorite examples of that because it can help support firmness, texture, and overall skin quality when it is done correctly and consistently.

RF Microneedling Is Not Basic Microneedling

Traditional microneedling works closer to the surface of the skin. RF microneedling adds radio frequency energy, which allows the treatment to work deeper. The tiny needles create controlled channels while the radio frequency energy delivers heat below the surface, where collagen remodeling can be encouraged.

The Heat Question: Is It Melting Fat?

A lot of people have heard concerns about heat-based devices and facial volume, so I want to address that directly. When RF microneedling is performed correctly, with the right device, depth, settings, and provider judgment, the goal is to stimulate collagen and improve skin quality, not remove volume where you want to keep it.

This is why technique matters. At Wake Up Beautiful, the device, settings, depth, placement, and candidacy all matter. It should never be a casual, one-size-fits-all treatment. The goal is to tighten, smooth, and strengthen the skin in a way that makes sense for the person in front of us.

It Can Treat More Than The Face

If microblading makes you nervous, this is the part I want you to hear first.

If you have ever considered microblading, nano brows, powder brows, ombre brows, or any type of permanent makeup, but stopped because you have seen brows that look too dark, too harsh, or obviously tattooed, I completely understand. Brows sit right in the center of the face. You should feel cautious about who touches them.

But here is the thing most people get wrong: not all permanent makeup is the same. The final result depends on the pigment, the tool, the technique, the skin, the design, and most importantly, the artist holding the tool.

Microblading Is A Technique, Not The Whole Category

Microblading is one way to implant color under the skin. Nano brows, powder brows, and ombre brows are other techniques. They all live under the larger umbrella of permanent makeup, which is really the art of placing pigment in the skin to create a soft, polished result.

So instead of getting attached to one technique name, I want you to think about the result you actually want. Most people are not really asking for microblading itself. They are asking for natural-looking brows, better symmetry, soft definition, and the feeling of waking up already put together.

After more than 24 years in this industry, I am not married to one brow technique for every person. Every face is different. Every skin type is different. Nasolabial folds are usually not the real problem.

Nasolabial folds are the lines that run from the nose toward the mouth, and they are one of the most common areas people ask about after 40. I understand why. They can make the face look heavier or more tired, even when you still feel youthful.

But here is the important part: the fold itself is often the result of something else happening above it. We lose support in the cheeks, temples, and lid-cheek area. Skin begins to loosen. Volume can shift downward. So if we only chase the fold, we may miss the real reason it is showing up.

Why Filling The Fold Alone Can Look Heavy

A little filler directly in the fold can be appropriate for the right person, but if we keep adding volume there without restoring the support above it, the lower face can start to look heavy, wide, or unnatural. That is the overfilled look nobody wants.

This is why I always come back to strategy. We want the right tool, in the right place, in the right order. The goal is not more. The goal is better.

Step One: Restore Structure Higher In The Face

Most of the time, the starting point is higher than the fold. We look at the mid-face, cheeks, temples, and the areas where support has been lost or shifted downward. When that structure is restored thoughtfully, the fold can soften more naturally.

This is how you avoid relying on filler to do all the heavy lifting. You are supporting the face instead of simply stuffing a line.

Step Two: Consider Lift And Support

Depending on the person, treatments like PDO threads may be part of the conversation because they can support a lifted look and reposition tissue in a subtle way. They are not for everyone, which is why consultation matters, but for the right candidate they can be a beautiful part of the plan.

Even if we restore structure and support, the skin itself still matters. If the skin is loose, dull, or crepey, you may not get the smooth, refreshed look you are imagining from filler alone.

4 Things I’d NEVER Do To My Face After 25 Years In Aesthetics

After 25 years in aesthetics, there are a few things I would not do to my own face.

Think of this as your well-informed girlfriend in the aesthetic space pulling you aside for a very honest conversation. I love beautiful treatments. I love great results. But I also know that not every trend, product, or shortcut is going to give you the refreshed look you actually want.

The goal is not to do everything. The goal is to do the right things, in the right order, for your face.

Number One: I Would Not Chase Sagging With Too Much Lower-Face Filler

When we start to see laxity around the lower face, jawline, or smile lines, it can be tempting to fill the area directly. I understand why. You see a crease or heaviness and want it softened.

But if the real issue is loss of cheek support, temple volume, lid-cheek support, or tissue descending from above, adding more weight to the lower face can make things look wider, heavier, or boxier. We want soft lift and balance, not a square or bulky look.

Number Two: I Would Not Chase Trends Just Because They Are Popular

Aesthetic trends come and go. Some are fun, some are overhyped, and some simply do not age well. Your face is not a trend board. The goal is not to look like someone else for a season.

Timeless results will always win. Refreshed, lifted, radiant, and still you is the lane I want for you.

Number Three: I Would Not Use The Wrong Tool For The Job

Not everything is a filler problem. Filler can be amazing when volume is truly the issue, but volume loss is not the only thing that makes us look older or tired. Sometimes the issue is skin quality. Sometimes it is collagen loss. Sometimes it is laxity.

If you are doing Botox and filler but still look tired, you may be missing the skin-quality piece.

This is something I hear all the time. Someone is doing Botox. They are doing filler. They feel like they are doing the right things, but they still look tired and cannot figure out why.

Let me break this down simply. Facial rejuvenation has three big parts: movement, volume, and skin quality. Botox and other neuromodulators address movement. Filler addresses volume. But skin quality is the canvas, and it is the piece many people are missing.

Botox And Filler Do Not Fix Everything

Botox can soften expression lines. Filler can restore volume and support. Those are wonderful tools when they are needed. But they do not fix sun damage, dullness, crepey texture, rough skin, redness, or that lackluster look that makes the whole face feel tired.

So you can be doing the right injectable treatments and still feel like something is off if the skin itself needs attention.

Skin Quality Is The Canvas

When the skin is healthy, smooth, glowing, and supported, everything else looks better. Your Botox can look better. Your filler can look better. Your whole face can look more refreshed because the surface is reflecting light differently and the tissue looks healthier.
